When publishing your iPhone/iPad app you can select which countries/territories. You can add or remove territories at https://itunesconnect.apple.com. Log in and click “Manage Your Applications”.
Select the app and locate “Rights and Pricing” in the right menu. The top blue button.
Here you can set both the availability date, price and more. Bottom view you can select the territories it will be available in.
